Going to a doctor’s office appointment can be difficult for people who are elderly or living with a debilitating condition that keeps them home-bound. To ensure patients stay healthy, we can provide care where patients need it most—in their homes. Our house calls program offers personalized, in-home wound management to patients with a chronic disease or recovering from a recent illness or hospitalization. On call 24-hours a day, our providers collaborate with each patient’s primary care provider to provide effective and seamless care. We provide supportive services to manage wounds; schedule diagnostic tests and therapies; educate patients about taking proper wound care; and facilitate communication among all members of the health care team, including the patient and family. Home visits are ongoing until the patient’s wound heals and he or she can comfortably resume regular doctor’s office visits.
